About Us
Proactive FQHC is a nonprofit organization working with Federally Qualified Health Centers and Tribal and Urban Indian Health Centers to improve how proactive screening and early detection are carried out in community health settings. As a mission-driven organization, we focus on practical, scalable approaches that support patient care while informing preventive care incentives through engagement with policymakers.

Preventive services help you stay healthy, proactively detect health problems early, determine the most effective treatments, and prevent certain diseases. Preventive services include exams, lab tests, and screenings. They also include programs for health monitoring, and counseling, and education to help you take care of your own health.
