Proactive FQHC is a nonprofit organization working with Federally Qualified Health Centers and Tribal and Urban Indian Health Centers to improve how proactive screening and early detection are carried out in community health settings. We focus on practical, scalable approaches that support patient care while informing preventive care incentives through engagement with policymakers.

Our work focuses on areas where health centers can test innovative approaches, close care gaps, identify what support is needed for broader adoption, and remain strong and sustainable.

We are exploring how hybrid prenatal care, home-based touchpoints, and remote monitoring can support access and continuity for community health center patients. Our early work focuses on practical workflow questions, patient engagement, and implementation needs.

Proactive Screening and Monitoring

Proactive FQHC promotes this important effort by the California Department of Public Health (CDPH) to maintain people's health, quality of life and their ability to work. CDPH endorses the recommendations provided by the United States Preventive Services Task Force (USPSTF) as they stood on January 1, 2025. CDPH together with other states, the strong public health and medical communities stand together in this common effort to share the science-based preventive screening and early detection recommendations people need to make healthy choices and protect our communities.

Policy Engagement

Proactive FQHC works with policymakers and community partners to bring health center priorities into conversations about care delivery, funding, and innovation. Our major policy priorities include protecting health center funding, 340B program policies, supporting health workforce innovation, strengthening reimbursement for preventive care, and advancing telehealth and remote monitoring.
