Community health centers serve 52 million Americans, nearly 1 in 7 people nationwide and 1 in 3 residents in rural communities. They provide primary care to 14% of the U.S. population while accounting for roughly 1% of total health care spending, making them one of the most effective and efficient parts of our health system. At their best, health centers are also a frontline for prevention, helping patients catch problems earlier and stay connected to care. Yet these organizations often operate under significant constraints, including limited resources, complex reimbursement structures, and growing demand.

Good ideas and innovative technology exist, but they often don't reach community health settings in a usable, sustainable way. We bridge that gap through pilots, implementation support, real-world testing, and advocacy, working with health centers to test approaches that support earlier intervention, stronger follow-up, and more proactive care.
