Community health centers serve 52 million Americans, nearly 1 in 7 people nationwide and 1 in 3 residents in rural communities. They provide primary care to 14% of the U.S. population while accounting for roughly 1% of total health care spending, making them one of the most effective and efficient parts of the health system.

Yet these organizations often operate under significant constraints. Limited resources, complex reimbursement structures, and growing demand can make it difficult to pilot new models, strengthen prevention, or adopt tools that support patient monitoring and more proactive care.
